An Advance Health Directive (AHD) is a way to make your wishes known about the health care and treatment you would – or would not – want if there comes a time when you can't speak for yourself or make your own decisions.
Creating an AHD gives you the opportunity to think about what matters most to you, including your values and preferences for future health care. It provides guidance for your loved ones and health professionals, helping them understand your wishes during what can be a stressful and emotional time. Your AHD is a legally binding document and becomes your voice when you need it most. Completing it is an important part of advance care planning.
Join us for an informative and engaging session where you'll be guided through the document. This process can help you clarify what's important to you and ensure your wishes are understood. You'll receive an Advanced Health Directive pack on the day.
